Commendation Incident Report: Special recognition may be in order whenever an employee performs his/her duties in an exemplary manner. This form provides a tool to document the general guidelines for commending exceptional employee performance.

Commendations and Awards Procedure: In order to review exemplary acts fairly and consistently, the department has established an Awards Procedure Guide. The guide lists criteria for multiple awards ranging from a simple letter of commendation to: Patrolman Jan Mulder II Award of Valor Detective Robert C. Taylor Excellence in Investigations Lieutenant Noah D. Studebaker Excellence in Supervision Depending on the circumstances, incidents are reviewed by supervisors, commanders, and an internal Awards Committee to ensure the proper award is bestowed and to maintain the integrity of the awards procedure.
